Stands the Test of Time; A Fist-Raising Manifesto

Robert Mendelsohn, M.D. was a physician in Chicago who put his patients and their lives ahead of "playing the game" and supporting/reinforcing the status quo of modern medicine. This book is a raging manifesto against the medical practices that are still strongly in play despite "patient-centered" practice and similar buzzwords prevalent today, revealed to be a new version of smoke and mirrors in the skewed power differential of the doctor-patient relationship. Each chapter is riveting, relevant, elegant, and thought-provoking, titled with common arguments used to discredit, question, or disempower patients. From a deep distrust of Big Pharma (before it was a name) with its mood elevators, tranquilizers, and oral contraceptives, Mendolsohn stands as a questioning, agitating force to protect women's reproductive systems and physical and emotional well-being. The names and players may have changed, but the intrinsic danger to women as primary health consumers is a powerful message. In light of today's class action lawsuits surrounding oral contraceptives such as Yasmin and its two billion dollar-a-year profit margin for Bayer, Mendelsohn's chapter titled "It's Safer Than Pregnancy," is particularly compelling as it exposes the flawed foundation of oral contraceptive safety testing and its global financial foundations, all framed within the drug's profound health impacts. Quote: "The estrogens affect every cell, every organ in the body. The undesired effects on other organs also vary from one user to another, but they include acute discomfort, serious disease, and even untimely death." "What Do You Need a Uterus For, Anyway?" addresses the issue of hysterectomy. "It's You Your Husband Loves, Not Your Breasts," historically examines Halstead's radical mastectomy and the fallout from said surgery. However, the first chapter titled "Trust Me, Dear" lays the book's foundation as it addresses the fundamental sociological/gender issues that encourage women to suspend their judgement and critical thinking when it comes to receiving and participating in medical care. This book runs the gamut from dangers of excessive radiation exposure ("We'll Just Take a Few Pictures"), overuse of pharmacological substances ("Take These and You'll Feel Better"), the inherent power differential ("What Medical School Did You Go To?") and multiple chapters addressing the dangers with excessive intervention in childbirth ("Do You Want Your Baby To Die?") "Male Practice" is a comprehensive and philosophical evaluation of the practice of medicine and the risks of participating in medical care, particularly for female consumers. (His "Confessions of a Medical Heretic" reveals a more general message regarding medicine and is another must-read). A stalwart classic that will have you asking questions that may have uncomfortable answers.

Still relevant today

I read Male Practice twenty-one years ago, because the book caught my eye and I had already been thinking about how doctors were much more apt to prescribe medications for women than for men. If a man experienced depression he had counseling prescribed for him. If a woman had the same problem, she would get a prescription for some kind of medication. I was also aware how insensitive society has been to basic anatomical facts about women: for example, making high-heeled shoes attractive to them, and discouraging them from speaking at a pitch that would be more natural for their vocal chords but instead pressuring them in all sorts of subtle ways to pitch their voices into the men's voice range no matter that it eventually ruins their voice.
I saw much more than that sort of thing in Dr. Mendelsohn's book, however. The shameful way women were treated in their unique role of pregnancy and childbirth was something I had not noticed before. Even now, obstetricians direct women to the table. That, and the stirrups, tells us we're still in the dark ages. Maybe not even that. In the dark ages women were using birthing stools, which are still the right way to go. He aptly points out that where men got control of childbirth, out went the birthing stool and in came a bunch of unnecessary problems, especially the dreadful episiotomy. Since a baby's weight is often resting on its umbilical cord during a supine childbirth, treatments for fetal distress are made necessary, including caesarian section (which I actually saw recommended when my son's head was crowning). The reclined, leaning back, lithotomy position that is currently being promoted for childbirth, is just about as bad. The influence of the male-dominated medical profession is visible in supposedly midwife-controlled birthing centers, where women's childbirth education is routinely channeled along male-approved lines. Even women gynecologists and obstetricians still toe the male-approved line, a fact pointed out when the book was written, so you can't count even on a woman doctor to look out for your best interests if you are a woman.
There is a lot of progress that is just as overdue now as it was when Dr. Mendelsohn wrote his book.
